چکیده

Respiratory problems contribute significantly to the overall morbidity and mortality in neurologically impaired children.1-3 Plioplys3 studied severely disabled children living in institution in United States and found that 77% of deaths were result of pneumonia. Aspiration, either from secretions/food in the pharynx or from reflux of gastric contents, is a very common phenomenon in this group of children.1,4 The true prevalence of aspiration pneumonia is difficult to determine because of the vague definitions and varying levels of clinical recognition. Although aspiration syndrome is very common, there remain many pitfalls in its management, including failure to distinguish aspiration pneumonitis from aspiration pneumonia, failure to recognise the spectrum of pathogens, and the misconception that aspiration must be witnessed. A physician survey in a university medical centre5 found out that there is divergent approach to the treatment of cases of aspiration with empirical antibiotics frequently initiated to treat noninfectious stages of aspiration. This article will review on the pathophysiology, clinical features and management of aspiration pneumonitis and aspiration pneumonia, with special emphasis on aspects related to neurologically impaired children.

Mesh hiatal reinforcement in laparoscopic Nissen fundoplication for neurologically impaired children is safe and feasible

Fundoplication is a standard treatment for gastroesophageal reflux disease (GERD) in neurologically impaired children. However, it has a high recurrence rate due to wrap herniation and disruption resulting from the failure or enlargement of the esophageal hiatus suture site. In adult patients with GERD, reinforcement of the sutured esophageal hiatus with a mesh significantly reduces the recurre...

Pneumonitis and pneumonia after aspiration

Aspiration pneumonitis and aspiration pneumonia are clinical syndromes caused by aspiration. These conditions are clinically significant due to their high morbidity and mortality. However, aspiration as a preceding event are often unwitnessed, particularly in cases of asymptomatic or silent aspiration. Furthermore, despite the difference in treatment approaches for managing aspiration pneumonit...

Preoperative fasting for paediatric anaesthesia.

Preoperative fasting was introduced to reduce the risk and severity of aspiration pneumonitis. Adequate time (6h) must still be allowed before operation for solid foods to be emptied from the stomach. However, the overwhelming weight of evidence supports the practice of reducing the duration of the preoperative fluid fast for elective paediatric surgical patients [3, 15]. In children allowed fr...

While the risk factors for aspiration pneumonia are similar to those for aspiration pneumonitis, the 2 syndromes have different presentations. Aspiration pneumonia tends to occur in older patients or in those with neurological diseases, and the aspiration is not usually witnessed. Aspiration pneumonitis is more likely to occur in patients undergoing anesthesia or in those with acute drug and al...
